急性心肌梗死患者血浆miR-542-5p表达水平<br />与心肌损伤程度的相关性分析

摘要:

目的 探讨急性心肌梗死(AMI)患者血浆miR-542-5p表达水平与心肌损伤程度的相关
性。方法 选取重庆大学附属涪陵医院心血管内科于2019年5月至2021年5月收治的AMI患者108例为研
究对象,依据疾病严重程度分为Killip 1级组(n=35)、Killip 2级组(n=33)、Killip 3级组(n=26)、
Killip 4级组(n=14)。分别测定其血清学指标:肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳
酸脱氢酶(LDH)及超敏肌钙蛋白(hs-cTnI)含量以评估心肌损伤程度,同时测定血浆miR-542-5p
相对表达水平,利用Speaman相关性检验评估miR-542-5p与CK、CK-MB、LDH及hs-cTnI含量的相关
性。制作ROC曲线,评价miR-542-5p对急性心肌梗死发生的预测效能。结果 四组患者的CK、CK-MB、
LDH、hs-cTnI含量水平及miR-542-5p表达水平比较,均随Killip分级增加而显著增高,差异有统计学
意义(P<0.05);经分析得出,CK、CK-MB、LDH及hs-cTnI含量水平(r=0.508,r=0.507,r=0.501,
r=0.493)均与miR-542-5p表达水平呈正相关,差异有统计学意义(P<0.001)。ROC曲线下面积为0.656
(95%CI:0.604~0.712,P<0.001),约登指数最大值为0.196,对应最佳截断值为2.026,灵敏度0.592,
特异度为0.604。结论 急性心肌梗死患者的血清CK、CK-MB、LDH及hs-cTnI水平随心肌损伤加重而增
加,血浆中miR-542-5p水平同样升高,两者呈显著正相关,提示miR-542-5p可作为AMI患者心肌损伤程
度的标志物的辅助指标,为临床AMI患者的预防及诊治提供补充依据。

Abstract:

Objective To discuss the correlation between expression level of plasma miR-542-5p and
severity of myocardial injury in patients with acute myocardial infarction (AMI). Methods AMI patients (n=108)
were chosen from Department of Cardiovascular Medicine in Fuling Central Hospital of Chongqing City from May
2019 to May 2021. All patients were divided, according to AMI severity, into Killip grade 1 group (n=35), Killip
grade 2 group (n=33), Killip grade 3 group (n=26) and Killip grade 4 group (n=14). The levels of serum of creatine
kinase (CK), creatine kinase-MB isoenzyme (CK-MB), lactic dehydrogenase (LDH) and high sensitivity cardiac
troponin I (hs-cTnI) were detected for reviewing the severity of myocardial injury, and meanwhile the expression
level of plasma miR-542-5p was detected. The correlation among miR-542-5p and CK, CK-MB, LDH and hscTnI
were reviewed by using Speaman correlation test. The predictive efficacy of miR-542-5p to AMI incidence
was reviewed by ROC curve. Results The comparison in levels of CK, CK-MB, LDH and hs-cTnI and miR-542-
5p expression level showed that they increased significantly as the increases of Killip grades in 4 groups (P<0.05).
The results of Speaman correlation test showed that levels of CK, CK-MB, LDH and hs-cTnI were all positively
correlated to miR-542-5p expression level (r=0.508, r=0.507, r=0.501, r=0.493, P<0.001). The area under ROC
curve of miR-542-5p prediction of AMI was 0.656 (95%CI: 0.604~0.712, P<0.001), the maximum value of Youden
index was 0.196, optimal cutoff value was 2.026, sensitivity was 0.592 and specificity was 0.604. Conclusion The
levels of serum CK, CK-MB, LDH and hs-cTnI increases with the aggravation of myocardial injury, and the level
of plasma miR-542-5p also increases in AMI patients, and they has a significant positive correlation. The results
suggest that miR-542-5p can be used as an auxiliary index for verifying severity of myocardial injury, and provide
supplementary evidences for prevention and treatment in AMI patients.
