急性冠脉综合征患者PCI术后发生心血管事件的<br />危险因素分析

摘要:

目的 探讨急性冠状动脉综合征(ACS)患者经皮冠脉介入治疗(PCI)后发生心血管事件
的危险因素及预防。方法 选择2018年6月至2020年6月于青岛市市立医院收治的137例ACS患者,根据PCI
术后心血管事件的发生情况将其分为再发组(n=29)和对照组(n=108)。收集患者临床资料,采用多
因素Logistic回归模型分析ACS患者PCI术后心血管事件发生的危险因素。结果 所有患者术后均随访1年,
29例术后发生心血管事件,其中心绞痛11例,心肌梗死8例,心律失常6例,心力衰竭4例;单因素分析
结果显示,两组患者在性别、是否独居、心力衰竭、首次手术、体质指数相比,差异无统计学意义(P
>0.05);两组患者在年龄、高血压、高血脂、糖尿病、辅助用药、心功能分级、吸烟、饮酒、冠脉病
变支数相比,差异均有统计学意义(P<0.05)。多因素非条件Logistic回归分析显示,年龄、高血压、
高血脂、糖尿病、辅助用药、心功能分级、吸烟、饮酒、冠脉病变支数均是急性冠脉综合征术后心血管
事件发生的独立危险因素(P<0.05)。结论 ACS患者PCI后发生心血管事件是多种危险因素共同作用形
成,应及时予以治疗,以延缓疾病进展,提高疗效。

Abstract:

Objective To discuss the risk factors and prevention of cardiovascular events in patients with
acute coronary syndrome (ACS) after PCI. Methods ACS patients (n=137) were chosen from Qingdao Municipal
Hospital from June 2018 to June 2020. All patients were divided, according to incidence of postoperative
cardiovascular events, into relapsed group (n=29) and control group (n=108). The clinical materials were collected
from the patients, and risk factors of postoperative cardiovascular events in ACS patients were analyzed by using
Logistic regression model. Results All patients were followed up for 1 y after the operation, and cardiovascular
events occurred in 29 cases, including angina pectoris in 11 cases, myocardial infarction in 8 cases, arrhythmia in 6
cases and heart failure in 4 cases. The results of single-factor analysis showed that the difference in gender, whether
or not living alone, heart failure, first operation and body mass index (BMI) had no statistical significance between
2 groups (P>0.05), and difference in age, hypertension, hyperlipidemia, diabetes, adjuvant drugs, heart function
grades, smoking, drinking and the vessel number of coronary artery lesions had statistical significance (P<0.05). The
results of multi-factor non-conditional Logistic regression analysis showed that age, hypertension, hyperlipidemia,
diabetes, adjuvant drugs, heart function grades, smoking, drinking and the vessel number of coronary artery lesions
(P<0.05). Conclusion The occurrences of cardiovascular events are induce by the joint action of many risk factors,
and timely treatment should be conducted to delay disease progression and improve curative effect.
